Designate a Beneficiary

It’s important to designate a beneficiary so your 401(k) assets are distributed according to your preferences in the event of your death. This can prevent potential disputes among family members and ensure your loved ones are taken care of as you intended.

Important Note: Beneficiary elections for other benefit programs (like life insurance) do not carry over to the 401(k) Plan; you must make a distinct election.

  • Log onto Fidelity NetBenefits

  • Select the Profile icon, then click Beneficiaries.  

  • Enter your beneficiary's Social Security number (SSN), address, and date of birth.

Quick tip! You can check your previously designated beneficiary at the same spot.

A non-spouse primary designation:

If you are married and designate someone other than your spouse as the primary beneficiary, your spouse must consent, and a notarized form is required. As you step through setting up your beneficiary on Fidelity NetBenefits, a consent form will generate for you to print, obtain the necessary signatures, and return to Fidelity. 

Beneficiary without an SSN:

If your beneficiary does not have a US Social Security Number (SSN), enter their information and foreign address (if any). If you pass away and your beneficiary still does not have an SSN or Tax Identification Number (TIN), they will need to complete a W-7 Form. After completing the W-7 Form and receiving a TIN from the IRS, the process to transfer your Microsoft 401(k) account funds to your beneficiary can be completed.
