Plan type: High-deductible health plan with a Health Savings Account Key features:

  • Choose any provider you like, but you pay less when you use a provider in the extensive Premera Blue Cross Network

  • Option to save for future health care expenses through a tax-advantaged, Microsoft-funded Health Savings Account (HSA)

  • Includes prescription coverage

  • In 2025, the limit on an HSA contribution for an individual is $4,300; for an individual with family coverage, the limit is $8,550

Preventive care: 100% covered  Out-of-pocket costs: A deductible applies before the plan begins to pay for services, except for preventive care (covered at 100%)  Provider network: Premera Blue Cross (nationwide)  Cash from Microsoft: Microsoft contributes to your HSA

Choose this option if 

  • You are currently enrolled in the Health Savings Plan and want to continue seeing your current doctors and other doctors of your choice.

  • You want a Health Savings Account to help you save for future health care costs or pay for out-of-pocket health care expenses.

  • You want access to a national provider network versus a regional network limited to available states.

Plan type: Coordinated care from in-network providers primarily located on the Eastside of the Puget Sound region.

Key features:

  • A personalized, coordinated approach to your health care

  • Predictable out-of-pocket costs when you see Health Connect network providers

  • Access to providers in the Extended Network of Premera Blue Cross (though members may pay more out of pocket)

  • Health Connect Navigator to help find providers and answer questions

  • Emergency care services available anywhere in the world

  • Includes prescription coverage

  • Maternity bundle copay for routine maternity care plus a doula allowance

Preventive care: 100% coveredOut-of-pocket costs:

  • Copays for office visits within the Health Connect network

  • Care within the Extended Network is subject to a deductible and coinsurance

Provider network:

  • Health Connect network (including Eastside Health Network, Allegro Pediatrics, and the Living Well Health Center & Pharmacy)

  • Extended Network through Premera Blue Cross

  • Out-of-network care includes emergency room and ambulance services; additional care is covered at 50% of allowable charges after the deductible

Choose this option if

  • You and your covered family members reside within King or Snohomish counties in Washington State and receive most of your care on the Eastside of the Puget Sound region (near Redmond).

  • You want coordinated care and predictable out-of-pocket costs.

Plan type: Health Maintenance Organization

Key features:

 

  • Only available to employees that live in Washington or California.
  • You and your primary care doctor coordinate your care with other providers within the Kaiser Permanente network.
  • In most cases, services provided outside the Kaiser Permanente network are not covered, except for emergency care

  • Offers online access to schedule appointments, view medical records, and exchange secure email messages with your doctors

  • Includes prescription coverage

Preventive care: 100% covered

Out-of-pocket costs:

  • No deductible and limited coinsurance

  • Copays for most outpatient services except for preventive care

Provider network:

  • Kaiser Permanente (in 11 available states)

  • Kaiser Foundation Health Plan of Washington (Washington)

Choose this option if

 

  • Only available to employees that live in Washington or California
  • You and your covered family members receive most of your health care within the Kaiser Permanente service area.
  • You prefer to pay a fixed amount per office visit and for other outpatient care.

  • You prefer to work with your primary care provider to coordinate your health care.

  • Includes prescription coverage

  • Doula reimbursement

  • Preventive care: 100% covered

Out-of-pocket costs:

  • Copay only plan

  • No Deductible 

  • No coinsurance

Provider network:

  • United Healthcare Choice Plus (Select Plus for those in California) nationwide network, including the Living Well Health Center & Pharmacy

  • Out-of-network care includes emergency room and ambulance services; additional care is covered with higher copays and may be subject to balance billing from out of network provider

Choose this option if

  • You want predictable out-of-pocket costs.

  • Ability to shop for medical care by viewing copays on the Surest member site before a medical visit
