Accidental death and dismemberment (AD&D) insurance helps protect you and your family on and off the job, 24 hours a day, anywhere in the world.
This is an optional benefit offered by Microsoft (you can purchase coverage for yourself and/or your eligible dependents). The cost for AD&D insurance is determined by the coverage level you elect. You pay for employee-only coverage using pre-tax dollars and employee and family coverage using post-tax dollars. To learn more, go to Prudential.
Note: The amount of coverage you elect for yourself determines the amount that is available to your eligible dependents.
As a Microsoft employee, you can elect coverage from 1 to 10 times your annual base pay, up to a maximum of $1,500,000. When you reach 70 years of age, the available coverage amount is gradually reduced.
If you die in a covered accident, AD&D benefits are paid to your named beneficiaries. If a covered dependent dies, you are the beneficiary of any amounts paid from the Plan.
Accidental death benefits are payable if you - or an insured dependent - die as the direct result of a covered accident, as follows:
At your accidental death, your beneficiary will receive the full AD&D benefit amount.
At your insured dependent's accidental death, you will receive a payment based on your elected AD&D benefit and your family structure at the time of the death.
The Plan also pays full or partial benefits for certain serious injuries other than loss of life. All Plan payments are in a lump sum, unless otherwise indicated.
The information below shows the benefits payable to you in the event of accidental death or injury within 365 days after the date of the accident. A percentage of these amounts would be payable for similar losses suffered by a covered dependent.
Loss of life; sight of both eyes; speech and hearing in both ears; both hands; both feet; one hand and one foot; one hand and sight in one eye; one foot and sight in one eye.
Loss of sight of one eye; speech; hearing in both ears; one hand; one foot; thumb and index finger of the same hand (25%); loss of hearing in one ear (25%); coma (1% per month up to 100 months).
Your AD&D insurance changes automatically when your pay changes. If you want to change your multiple of coverage or coverage for your dependents, you can do so during the annual Open Enrollment.
Note: You have the right to apply for AD&D coverage for yourself or your dependents if your insurance ends because you leave the company.
You are eligible to participate in a Microsoft-sponsored insurance plan if you are in an approved budgeted headcount, regular employment position with Microsoft, on the Microsoft US payroll. Employees of subsidiaries or affiliates of the Microsoft Corporation that participate in the Plan are also eligible. Interns and visiting researchers are not eligible.
Full-time Retail Store Employees must work at least 32 hours per week and have completed 30 days of employment. Part-time Retail Store Employees must work between 20 and 31 hours per week and have completed 90 days of employment.
If both you and your spouse or domestic partner are employed by Microsoft and are eligible for benefit coverage, you can be covered as a dependent under your spouse or eligible domestic partner's Plan and enroll in your own separate employee AD&D insurance coverage. Your eligible children can only enroll under one employee's coverage.
You can enroll your eligible dependents in the Plan. Eligible dependents include:
Legally married spouse
Domestic partner (same or opposite sex)
Dependent children under age 26
Incapacitated dependent children age 26 or over
The following are not eligible to participate in the Plan as a dependent:
Legally separated or divorced spouse
Parents, siblings, nieces, nephews, grandchildren, and grandparents
Roommates
Foster children
Any person who is on active duty in the armed forces
Anyone for whom you fail to provide proof of eligible dependent status
If you have a qualifying status change during the Plan year you can make benefit changes within 30 calendar days from the date of the event. Qualifying status changes include:
Birth of a child
Legal marriage or initiation of a domestic partnership
Divorce or legal separation
Termination of a domestic partnership
Loss of a job by your spouse/domestic partner
