This program is designed to support you and your eligible family members in preventing and managing diabetes. We offer two individual programs at no cost to you.

This program is designed to support you and your eligible family members in preventing and managing diabetes. We offer two individual programs at no cost to you.
Whether you’re covered by a Premera plan or through Surest, our programs have two main components:
Diabetes Prevention: This program is for those at risk of diabetes and provides tools and support to help you manage your health and prevent diabetes.
Diabetes Management: This program is for those who need to manage type 1 or type 2 diabetes, offering comprehensive support and resources to help you monitor and control your condition.
Employees enrolled in the Premera Health Plans may be eligible for the Diabetes and Prevention Programs through Teladoc Health.
Employees enrolled in the Surest Health Plan may be eligible for the Diabetes and Prevention Programs through Virta.
You are eligible for our Diabetes Prevention Programs if you are a US benefits-eligible part-time or full-time employee, or an eligible family member, enrolled in a Premera or Surest health plan. Specific eligibility criteria apply for each program.

Premera members
Healthy Living and Diabetes Prevention
Eligibility: Premera plan members 18 years and older who meet pre-diabetes criteria as determined by the CDC.
Program Duration: 12 months, with an additional 12 months of access for maintenance.
Benefits:
Cellular-connected scale for tracking weight.
Real-time tips and personalized feedback on health, nutrition, and lifestyle changes.
Unlimited coaching and support via phone, text, email, or mobile app.
CDC-recognized weight management curriculum.
Periodic review and feedback from a health coach.
Experiential learning missions covering nutrition, activity, motivation, sleep, and stress management.
Mobile app for tracking weight, steps, and health goals.
Diabetes Management
Eligibility: If you or your family members 13 years of age and older need to manage type 1 or type 2 diabetes, you can access Diabetes Management from Teladoc Health.
Benefits:
A blood glucose meter that uses cellular technology to automatically upload blood sugar readings to a personal online account.
A lancing device and unlimited lancets at no cost to you.
Unlimited test strips for this meter at no cost to you. You can reorder test strips using the meter or online. The strips will be sent to you directly.
Real-time feedback and tips based on your blood sugar readings that can help keep your levels within a healthy range.
Coaching and support via phone, text, e-mail message, or the program manager's mobile app.

Coverage: Your benefits provide 100 percent of the charges for Healthy Living and Diabetes Prevention and Diabetes Management. Your fees for these programs aren't subject to a deductible and a coinsurance maximum doesn't apply. Remember that these programs don't cover drugs or surgical procedures to assist in reducing weight or lowering blood sugar or cholesterol.
Note: If for any reason your coverage terminates during the time you are participating in this program, and you do not elect COBRA, only services rendered up to the date of termination will be reimbursed.
Surest members
Diabetes Prevention
Eligibility: Surest plan members and family who are 18 years of age and older with an A1c between 5.7 – 6.4.
Benefits:
1:1 health coaching, motivational support, behavior-based guidance, personalized nutrition based on low-carb, moderate protein and high healthy fats, while customizing it to the member's individual budget, food preferences, cultural preference and lifestyle.
Virta provides supports and oversees intensive medication management, and the option to safely off-ramp medications while reversing their condition and sustaining weight loss.
Bluetooth connected weight scale and a Bluetooth connected glucometer
24/7 access to in-app texting, the Virta member community, resources, educational videos, and behavior change health content.

Diabetes Management
Eligibility: Surest plan members and family 18 years of age and older with an A1c of 6.5 or higher and a diagnosis of type 2 diabetes
Benefits:
With Diabetes Management, you can track and monitor your disease, order strips and lancets, and provide regular updates to family members.

Unlimited test strips.
Real-time feedback and tips based on blood sugar readings.
