Enrollment into Weight Management programs will depend on which health plan you’re enrolled into; if you’ve chosen to waive enrolling in a health plan, you are not eligible to participate in the programs available through Surest, Kaiser, or Premera. If you'd like to participate in one of the programs, you can enroll in one of the health plans during the Open Enrollment period, which takes place in the fall, unless you experience a Qualified Status Change.
Enrollment into the health plan-provided weight management programs also has physical requirements that need to be met as well.
If you’re interested in enrolling in the weight management program through Premera or Kaiser Foundation Health Plan of Washington, you need to meet one of the following requirements:
Have a BMI greater than or equal to o27 and are diagnosed with two or more of the following conditions: Congestive heart failure, coronary heart disease, depression, diabetes, high cholesterol, high blood pression, sleep apnea
If you’re interested in enrolling in the weight management program through Surest, you need to meet both of the following requirements:
There are no eligibility requirements for participating in coaching sessions through our wellbeing program or the Living Well Health Center.
